If you have not been to our clinic  before,  you will need to fill out our registration & intake forms before the appointment can take place. Please see the Patient forms tab above or follow the instructions while booking. If we do not receive the registration & intake forms your appointment may be cancelled.

 2024: masks are no longer mandatory in our office, we do kindly ask patients with respiratory symptoms to still wear a mask. A mask will be provided for you.
